Abstract

Software reliability assessment using operational failure reports is an important practice for evaluating how software performs under real working conditions. In enterprise systems, operational failures may include service interruptions, transaction errors, application crashes, database failures, slow response, exception events, and user-reported incidents. This article discusses how operational failure reports help teams measure reliability, identify recurring fault patterns, analyze affected modules, and understand the impact of failures on business operations. It explains the role of failure frequency, severity level, downtime duration, recovery time, root cause records, release history, and corrective action notes in assessing software dependability. The article also highlights common challenges such as incomplete failure reporting, duplicate incidents, unclear error descriptions, delayed root cause analysis, and difficulty linking failures to specific software changes. A structured reliability assessment approach is presented to improve failure classification, support maintenance planning, reduce repeated incidents, and strengthen release quality. The study concludes that effective use of operational failure reports improves software stability, supports better corrective decisions, and enhances the reliability of enterprise applications.
