Software Fault Analysis Using Defect Root Cause Reports

Software fault analysis using defect root cause reports is an important quality improvement practice for understanding why defects occur and how similar faults can be prevented in future releases. In enterprise software projects, faults may arise from unclear requirements, design mistakes, coding errors, configuration issues, weak testing, poor communication, or uncontrolled changes. This article discusses how defect root cause reports help teams identify the original source of software faults instead of only correcting visible failures. It explains the role of defect history, severity classification, affected modules, correction records, review findings, test evidence, and recurrence patterns in supporting fault analysis. The article also highlights common challenges such as incomplete defect descriptions, incorrect root cause assignment, delayed reporting, repeated similar defects, and lack of follow-up actions. A structured fault analysis approach is presented to improve defect prevention, strengthen process control, reduce rework, and support continuous software quality improvement. The study concludes that effective use of defect root cause reports improves software reliability, supports better decision-making, and enhances long-term quality management in enterprise software projects.
